By accessing or using this website, submitting information through the website, enrolling in an available membership, purchasing a service, or otherwise interacting with Optimum Care Solutions, Inc., you agree to these Terms and Conditions.
ABOUT OPTIMUM CARE SOLUTIONS, INC.
Optimum Care Solutions, Inc. provides integrated healthcare and professional services. Based on the services presented on this website, offerings may include behavioral health and substance-use care, psychiatric evaluations, medication management, individual therapy, substance-use evaluations and treatment, recovery and treatment support, care coordination, DOT and occupational health services, DOT medical examinations, CDL physicals, mobile and on-site services, employer and fleet health services, healthcare consulting and advisory services, and membership-based care options.
Services may not be available in every location. Availability may depend on the patient's or client's location, provider availability, professional licensing, applicable law, clinical appropriateness, employer requirements, and other factors.
WEBSITE INFORMATION
Information provided through this website is intended to provide general information about Optimum Care Solutions, Inc. and its services.
General website content is not a substitute for an individualized evaluation, diagnosis, treatment plan, medical advice, mental-health advice, substance-use treatment advice, or other advice from an appropriately qualified healthcare professional.
A provider-patient relationship is not established merely by visiting this website, completing a contact form, sending an email or SMS message, or requesting an appointment. A clinical relationship is established only through the processes required by Optimum Care Solutions, Inc. and applicable law.
MEDICAL EMERGENCIES
This website, its contact forms, email addresses, and ordinary SMS channels should not be used for medical emergencies.
If you believe you or another person is experiencing a medical emergency, call 911 or seek appropriate emergency medical care immediately.
If you or another person is experiencing a mental-health or substance-use crisis, use appropriate emergency or crisis resources available in your location.
APPOINTMENT REQUESTS
Submitting an appointment request does not guarantee an appointment or establish a provider-patient relationship.
Appointments are subject to provider availability, service availability, location, applicable licensing requirements, clinical appropriateness, completion of required forms, and other applicable requirements.
Optimum Care Solutions, Inc. may contact you to obtain additional information before confirming an appointment.
TELEHEALTH SERVICES
The website materials indicate that certain services may be available through telehealth.
Telehealth services may be subject to separate informed-consent requirements, clinical policies, technology requirements, identity verification, location verification, provider licensing restrictions, and other applicable requirements.
Telehealth may not be appropriate for every condition or circumstance. Your provider may determine that an in-person evaluation, emergency care, specialist referral, or another form of care is appropriate.
BEHAVIORAL HEALTH AND SUBSTANCE-USE SERVICES
Optimum Care Solutions, Inc. offers behavioral-health and substance-use services that may include psychiatric evaluations, medication management, individual therapy, substance-use evaluations and treatment, recovery and treatment support, and care coordination.
The particular services recommended or provided depend on individual circumstances, clinical assessment, provider judgment, applicable professional standards, and legal requirements.
No website statement guarantees a particular diagnosis, medication, treatment, recovery, clinical result, or health outcome.
PRESCRIPTIONS AND MEDICATION MANAGEMENT
Where medication-management services are provided, prescribing decisions are made by appropriately qualified healthcare professionals based on clinical judgment and applicable law.
Submitting an appointment request or participating in an evaluation does not guarantee that medication will be prescribed, continued, changed, or discontinued.
Prescription availability may be affected by clinical considerations, pharmacy availability, federal or state requirements, controlled-substance rules, telehealth requirements, and other applicable restrictions.
DOT AND OCCUPATIONAL HEALTH SERVICES
The website describes DOT and occupational health services including DOT medical examinations, CDL physicals, mobile and on-site services, employer and fleet health services, flexible service delivery, and related occupational-health support.
Completion of an examination does not guarantee medical certification, employment eligibility, continued employment, regulatory approval, or any particular examination result.
Determinations are based on applicable requirements and the findings of the appropriately qualified professional conducting the examination.
EMPLOYER AND FLEET SERVICES
Employers, transportation companies, owner-operators, fleet operators, and other organizations may engage Optimum Care Solutions, Inc. for occupational-health services.
The scope, pricing, responsibilities, scheduling arrangements, reporting requirements, and other terms for organizational services may be established through a separate agreement, proposal, statement of work, or invoice.
HEALTHCARE CONSULTING AND ADVISORY SERVICES
Optimum Care Solutions, Inc. also provides healthcare consulting and advisory services.
The website identifies consulting services including program development, operational excellence, compliance support, quality improvement, policy and procedure development, and strategic advisory services. Its consulting methodology is presented as Assess, Plan, Implement, Optimize, and Achieve.
Consulting services are advisory in nature. Clients remain responsible for their organizations, decisions, operations, personnel, regulatory obligations, policies, implementation, and professional judgments.
COMPLIANCE CONSULTING DISCLAIMER
Healthcare compliance and policy consulting may assist organizations with processes, documentation, quality initiatives, accreditation preparation, and operational practices.
Unless expressly provided by an appropriately licensed professional under a separate agreement, consulting services do not constitute legal, tax, accounting, or other regulated professional advice.
Clients remain responsible for obtaining legal or other specialized advice where appropriate and for determining whether their organizations satisfy applicable laws, regulations, accreditation standards, contractual requirements, and professional obligations.
MEMBERSHIP OPTIONS
The website materials describe Essential Care, Continuity Care, and Comprehensive Care membership options and indicate monthly pricing of $99, $179, and $279 respectively.
The precise benefits, eligibility requirements, service limits, exclusions, billing arrangements, and availability of each membership are governed by the membership terms presented at enrollment and any applicable patient or membership agreement.
A healthcare membership is not health insurance. Membership does not replace insurance coverage that an individual may need for hospital services, emergency care, specialists, medications, diagnostic testing, or other healthcare expenses outside the membership.
PRICING
Pricing depends on the service, membership, client type, scope, location, complexity, and other applicable factors.
Where a specific price is displayed on the website, that price applies subject to the accompanying description, eligibility requirements, limitations, and terms.
Healthcare consulting and organizational engagements may be priced on a per-project, hourly, recurring, retainer, milestone, or other mutually agreed basis.
Final pricing for customized engagements will be communicated before paid work begins.
PAYMENT TERMS
Payment may be required at the time of service, before an appointment, upon enrollment, according to a recurring membership schedule, upon invoice, or according to a separate written agreement.
For consulting and organizational services, unless different payment terms are stated in the applicable agreement or invoice, invoices are due within 30 days of the invoice date.
You are responsible for providing accurate billing information and paying authorized charges when due.
PAYMENT PROCESSING
Payments may be processed through Stripe or another third-party payment processor.
By submitting payment information and authorizing a transaction, you authorize Optimum Care Solutions, Inc. and the applicable payment processor to process the disclosed charge.
Payment processors may maintain their own terms, security procedures, and privacy practices.
RECURRING MEMBERSHIP PAYMENTS
If you enroll in a recurring membership, you authorize the applicable recurring charges according to the amount and billing frequency disclosed when you enroll.
Recurring charges will continue until the membership is canceled or terminated according to the applicable membership terms.
Any material membership-specific renewal or cancellation conditions will be disclosed as part of enrollment.
LATE PAYMENTS
Amounts not paid when due may result in suspension or termination of non-emergency membership, consulting, administrative, or other services where permitted by law and applicable professional obligations.
For invoiced consulting or organizational services, reasonable late charges and lawful collection costs may apply when disclosed in the applicable agreement or invoice. Any late charge will not exceed the amount permitted by applicable law.
CANCELLATION AND MISSED APPOINTMENTS
Appointment cancellation, rescheduling, and no-show requirements may vary by service and will be communicated through applicable scheduling or patient policies.
Patients and clients should cancel or reschedule as early as reasonably possible.
Late cancellations or missed appointments may be subject to a fee when that fee has been properly disclosed and is permitted by applicable law.
MEMBERSHIP CANCELLATION
Memberships may be canceled according to the cancellation procedure disclosed at enrollment.
Cancellation generally prevents future membership renewals after the cancellation becomes effective but does not automatically create a refund for a membership period that has already begun or for services already provided.
REFUND POLICY
Fees for healthcare services already rendered are generally nonrefundable.
Membership charges for an active membership period are generally nonrefundable once benefits or services for that period have become available, except where otherwise required by law or provided in the applicable membership agreement.
For consulting services, fees attributable to work already performed are generally nonrefundable. If prepaid consulting services are canceled before substantial work begins, Optimum Care Solutions, Inc. may provide a full or partial refund after deducting amounts attributable to work completed, committed resources, authorized expenses, and nonrecoverable third-party costs.
Duplicate charges, payment errors, or other exceptional billing issues may be reviewed individually.
INSURANCE AND BENEFITS
The materials provided do not establish specific insurance plans accepted by Optimum Care Solutions, Inc. Accordingly, patients should confirm insurance, reimbursement, benefit, and payment arrangements directly before relying on insurance coverage for a service.
Insurance coverage and reimbursement decisions are ultimately determined by the applicable insurer or benefit administrator.
PATIENT RESPONSIBILITIES
Patients are responsible for providing accurate and complete information reasonably necessary for their care, including relevant health information, medications, allergies, symptoms, medical history, and other information requested by their healthcare professionals.
Patients should ask questions when they do not understand information concerning their care and should follow emergency instructions when urgent assistance is required.
NO GUARANTEE OF HEALTH OUTCOMES
Healthcare outcomes vary from person to person.
Optimum Care Solutions, Inc. and its providers do not guarantee any particular diagnosis, treatment response, medication result, recovery outcome, occupational certification, or improvement in health or well-being.
TESTIMONIALS
The website may contain testimonials describing individual experiences with Optimum Care Solutions, Inc. The supplied website materials include testimonials concerning behavioral healthcare, medication management, telehealth, DOT physicals, and other experiences.
Testimonials represent individual experiences and do not guarantee that another patient or client will receive the same result.
INTELLECTUAL PROPERTY
Website content, branding, text, graphics, designs, logos, educational materials, consulting frameworks, and other proprietary materials are owned by or licensed to Optimum Care Solutions, Inc. unless otherwise stated.
You may not reproduce, distribute, modify, sell, or commercially exploit protected materials without authorization.

Message frequency varies. Message and data rates may apply.
Consent to receive SMS messages is not a condition of purchasing goods or services or receiving healthcare.

You may opt out of SMS communications at any time by replying STOP.
After your opt-out request is processed, you will no longer receive nonessential SMS messages under that consent unless you subsequently opt in again.

SMS CONSENT AND DATA SHARING
SMS consent and mobile information collected for SMS purposes will not be sold, rented, or shared with third parties or affiliates for their marketing or promotional purposes.
Service providers may process SMS information when reasonably necessary to facilitate communications on behalf of Optimum Care Solutions, Inc. Such providers are not authorized to use SMS consent information for their own independent marketing purposes.
COMMUNICATION PRIVACY
Ordinary email, website forms, and SMS may not be appropriate for highly sensitive medical information unless Optimum Care Solutions, Inc. has specifically designated the communication method for that purpose.
Patients should use designated secure systems when instructed to do so.
PRIVACY AND HEALTH INFORMATION
Our general website privacy practices are described in our Privacy Policy.
Protected health information created or maintained in connection with healthcare services may also be subject to additional privacy requirements and notices, including any applicable Notice of Privacy Practices.
THIRD-PARTY SERVICES
The website or services may use third-party scheduling systems, payment processors, telecommunications providers, patient portals, technology vendors, laboratories, pharmacies, or other independent services.
Third-party services may be governed by separate terms and privacy practices.
LIMITATION OF LIABILITY
To the fullest extent permitted by applicable law, Optimum Care Solutions, Inc. will not be liable for indirect, incidental, special, consequential, exemplary, or punitive damages arising solely from use of the general website or nonclinical consulting services.
Nothing in these Terms and Conditions is intended to waive, disclaim, or limit rights or liabilities that cannot lawfully be waived, disclaimed, or limited, including applicable rights concerning professional healthcare services.
